The Director - RN, Case Management acts as an operational case management liaison with the vendor and works to connect appropriate teams in support of requests, initiatives, etc. Maintains and leverages ongoing business relationships with internal departments to facilitate business goals and objectives, including but limited to: Analytics, Ambulatory Care Coordination Team and Post-Acute Leadership Team.
Primary Responsibilities
Acts as an operational liaison between select out-of-scope SSM Health teams; provide coordination across the care continuum with focus on case management.
Manages internal and external stakeholder relationships to communicate progress, risks, issues, and mitigation to Care Transformation (CT). Leads to proactively drive resolution.
Escalates issues, risks, disputes to the CT leadership, as appropriate.
Maintains knowledge of latest best practices and industry insights related to case management for purposes of continued strategic planning and innovation. Support CT leadership in strategic planning, innovation, goal setting and other areas.

Maintains an ongoing awareness of select regulatory requirements to ensure compliance such as: CoP for discharge planning, CMS two midnight rule, Department of Health and Senior Services requirements, state regulatory requirements. Maintains flexibility and alignment with Vendor to accommodate changing organizational and industry requirements.
Recruits, motivates, develops, and retains a superior functional leadership team. Implements a management system that is customer-focused, encourages efficiency and automation, drives performance, and effectively develops and rewards key
Works in a constant state of alertness and safe manner.

Requirements
Education
Bachelor’s Degree
Experience
Seven years with five in direct people management/leadership
Required Professional License and/or Certifications
Registered Nurse (RN) - Oklahoma Board of Nursing (OBN) or Registered Nurse (RN) Issued by Compact State
