Description
Chief Population Health Officer
Michigan Medicine / University of Michigan Health
Ann Arbor, Michigan
This is an exciting opportunity for a visionary architect to champion Michigan Medicine’s transformation to value-based care, leveraging robust assets in data, contracts, and care management. With dedicated startup infrastructure support, the Chief Population Health Officer will lead system-wide and statewide population health strategy across a premier Academic Medical Center and an expanding Regional Health Network.
The Opportunity
The Chief Population Health Officer (CPHO) will focus on how to best provide care to the patients and the broader communities served by the system, including care outside of traditional facilities and integrating transitions across elements of care that will enable the health system’s success in value-based payment arrangements. This leader will be charged with assessing the current program landscape and designing a forwardlooking structure that positions the system for success now and for the future.
Additionally, the CPHO oversees performance management of value-based payment models, including payer quality and care management incentive programs, and ensures success by developing and implementing innovative health care delivery models and team-based care in collaboration with other key health system leaders. The CPHO will also play a key role in identifying, developing, and implementing new value-based care business opportunities.
The Ideal Candidate
This leader will ideally have experience in a complex academic health system and have demonstrated success leading by influence in a matrix environment. The CPHO will have proven experience as an architect of strategy, capable of unifying fragmented operational pieces into a coherent plan, and progressively responsible experience in the management of health care delivery. The CPHO will have successfully led significant transformation in a large academic or similar complex health system environment and will hold an MD, DO, PharmD, DNP, or equivalent clinical credential.
